Medical/Inactivity Re-Rate Requests

If you are requesting a medical re-rate or inactivity re-rate (as per Rules and Regulations 2.04), your request must be sent to the Director of Eligibility no later than ten days prior to the tournament in which you are playing. For medical re-rates, you must complete the form found in the Documents section and follow the instructions provided.
